The survey linked below collects the information our team needs to begin the prior authorization process for TMS or Spravato® services. It includes sections for your demographic details, current and past medications, insurance information, and a symptom severity questionnaire.

Please have your insurance card available and allow approximately 15 minutes to complete the form. Once submitted, your information will be securely sent to our staff, who will initiate the prior authorization and contact you once we receive a response from your insurance company.
